Background: ADHD is associated with substantial impairment and elevated risks of severe real-world adverse events across the lifespan (e.g., suicidal behavior, motor vehicle crashes, substance misuse, criminality, and premature mortality). While randomized trials establish short-term efficacy for symptom control, evaluating long-term real-world risk reduction requires synthesizing evidence from diverse observational and causal-inference designs.
Aim: This narrative review synthesizes current evidence on the association between ADHD pharmacotherapy and major real-world functional outcomes, balancing behavioral risk reduction against potential long-term cardiovascular safety concerns.
Methodology: We searched PubMed, Web of Science, Embase, and PsycINFO for studies on stimulant and non-stimulant treatments. Evidence from randomized trials, register-based studies, within-individual comparisons, and target-trial emulations was synthesized narratively, focusing on how study design affects causal inference.
Results: ADHD pharmacotherapy is associated with lower rates of suicidal behavior, substance-related problems, transport accidents, criminal convictions, psychiatric hospitalizations, and premature mortality. Conversely, long-term cumulative exposure—particularly to stimulants—is linked to increasing cardiovascular risk, especially hypertension, with stronger associations at longer durations and higher doses.
Conclusions: ADHD pharmacotherapy contributes to critical real-world risk reduction beyond core symptom control. Clinical decisions should balance these behavioral and survival benefits against potential long-term cardiovascular risks through ongoing safety monitoring and individualized benefit-risk assessments.
